Learn about ways to assess the causes of diagnostic errors.

Many studies of diagnostic errors have concluded that cognitive and systems factors are often both present and interact to lead to errors. We have highlighted some important studies examining the causes of diagnostic error and have categorized them into mainly cognitive-focused vs. mainly system-focused, recognizing that some overlap remains.

Cognitive

These studies focus mainly on individual providers and their decision-making. Please see the SIDM website for an introduction to clinical reasoning.

System

  • General
  • Diagnostic tests
  • Follow-up and tracking
  • Communication/Coordination of care
